Request Resources

There is a limited amount of resources available for order. Please know that the submission of a resource request does not guarantee your order will be filled. Orders will be prioritized based on critical healthcare and current supplies.

Request supplies for schools (PPE kits, disinfectant and cloth masks)

Maricopa County worked with area school districts and others to distribute more than 1,200 PPE Starter Kits to public, private and charter schools. Kits include:

  • 50 Surgical Face Masks 
  • 10 Face Shields
  • 100 Pairs of Gloves
  • 10 Gowns (5 re-usable & 5 disposable)
  • 1 No-Touch IR Thermometer
  • PPE Care Flyer
  • Donning and doffing guide

Free cloth face masks, classroom disinfectant and hand sanitizer for use by students or staff are also available. Use the request button below to submit supply requests.

PPE

Personal Protective Equipment (PPE) requests can include N95 respirators, surgical masks, face shields, and isolation gowns.

If you are a Private Practice or Private Doctor’s Office please go to the Maricopa County Medical Society for requesting PPE. Click here for their Request Form

Before submitting a PPE request you must prove that you have taken the following steps:

  1. Follow CDC’s guidance on optimizing PPE Supplies
  2. Reach out to multiple vendors. If vendors cannot supply your order, keep the documentation.
  3. Reach out to other partners in the area. Many jurisdictions are already sharing with each other.
  4. If you have completed these steps you can submit a healthcare order request through the link below.

COVID-19 Testing Kits

The CDC recommends using authorized nucleic acid or antigen detection assays that have received an FDA EUA to test persons with symptoms when there is a concern of potential COVID-19. Tests should be used in accordance with the authorized labeling; providers should be familiar with the tests’ performance characteristics and limitations.

Before submitting a request for COVID-19 Testing Kits you must prove that you have taken the following steps:

  1. Review CDC’s Overview of Testing for SARS-CoV-2Please make sure you are testing in accordance with CDC guidelines.
  2. Reach out to multiple vendors. If vendors cannot supply your order, keep the documentation.
  3. Reach out to other partners in the area. Many jurisdictions are already sharing with each other.

Request Ventilators

During a large-scale public health emergency involving a respiratory disease like COVID-19, stockpiled ventilators will be deployed in a way that optimizes the effectiveness, efficiency, and equity of this scarce resource.

Before submitting a request for Ventilators you must prove that you have taken the following steps:

  2. Determine your need. Consider the number of and preferred type of ventilator. Please refer to the Ventilator Request Guidelines for a complete list of considerations.

Request Emergency Staffing

Maintaining appropriate staffing in healthcare facilities is essential to providing a safe work environment for healthcare personnel and safe patient care. Healthcare facilities must be prepared for potential staffing shortages and have plans and processes in place to mitigate these, including communicating with healthcare personnel about actions the facility is taking to address shortages and maintain patient and healthcare personnel safety.

Before submitting an Emergency Staffing Request you must prove that you have taken the following steps:

  1. Follow CDC’s Strategies to Mitigate Healthcare Personnel Staffing Shortages.
  2. Reach out to multiple vendors. Contact AzCHER at SMurphy@azhha.org or call 928-542-3182 for an updated rapid deployable COVID-19 Vendor List.
  3. Complete Mission Request Profile Considerations. Work with your leadership to determine the type of staffing, paid status, lodging capacity, etc. Please refer to the Workforce Request Guidelines for a complete list of considerations.
